1. Statement of pork free facility (see enclosed)
2. Evidence of Halal Policy dissemination
Top Management must establish written halal policy which indicates the company's commitment to produce halal products consistently and the halal policy must be disseminated to all
stakeholders by training, banners, information board (sample of banners/information board about halal policy please see enclosed).
3. Evidence of HAS Internal Training execution --> QA will provide Internal Halal Training on Week 3 of October
4. Evidence of HAS Internal Audit execution --> see the checklist enclosed. QA will verify and final check on week 3 of October
5. Halal Management Team (see enclosed)
6. Training both internal and external (Procedure, Schedule, Evaluation, Evidence of training implementation)
7. Purchase order of Raw Material minimum for 3 months before Halal Indo audit.
8. Procedure of Critical process
a. Incoming raw material --> must include inspection regarding halal aspect such as check the suitability between actual raw material (name of product, manufacturer name, country of origin)
and list halal of Raw material Mayora Group
b. Production procedure --> must include inspection regarding halal aspect such as check the suitability between actual raw material (name of product, manufacturer name, country of origin)
and list halal of Raw material before using the raw material in the production line.
c. Cleaning procedure --> must include verification after cleaning regarding halal aspect such as, verify all equipment and machine are free from dirt and filth.
d. Purchasing procedure (selecting, purchasing raw material) --> corporate procedure
e. R&D procedure --> corporate procedure
f. Non-conformity product & process procedure --> must include how to handle products that not meet Halal criteria.
Please add this statement on the procedure : In processing products and finished products which contaminated with haram/forbidden product or filth, must be destroy or follow the
provisions of LPPOM MUI that is used for consumer products that not require Halal requirements and must be repack with packaging material without halal logo.
g. Traceability procedure
h. Internal audit --> the procedure must include about halal internal audit, schedule/frequency, method, and the report must be sent to coordinator Internal Management Halal of Mayora
Group every 6 months along with the halal periodic report.

recorded.
